0000-462 POSITION SUMMARY The Case Manager Family Peer Support Specialist works to intervene through early identification and supportive advocacy to identify needs and connect families with the "right sized" services. FPS provides parents with the following program supports: Referrals, Needs Assessments, Goal Setting, Linking with Behavioral Health, Child Welfare and Community Resources, and Providing Peer Support and Parent Training to promote Positive Parenting and Build Capacity. FPS specialist will provide parenting information , will advocate as an additional voice for the family, offer support and understanding as someone who "has been there, done that", coordinate parent groups, and assist in informal networking to help develop a sustainable and understanding support system.

ESSENTIAL JOB FUNCTIONS:

Meet with clients: individually or in a group to assess needs of the clients; collaborate with clients, families and other case managers and program staff. Assess progress per agency policy, regulatory requirements and best practices. Assure documentation and record keeping is in compliance with contract, regulatory and agency requirements and best practices. Develops and/or implements individualized client treatment or service plans and ensures compliance with applicable standards and agency policies. Efficiently operate personal computer to enter, edit and/or retrieve data and to produce reports and correspondence. Empower parents to create a support system with usable resources Effectively works as a team member, including participating in all required trainings, ensuring the safety of others and participating in Peer Review/CQI process. Provide support and assistance to families as a resource, mentor and advocate for the needs of the parents and their families Meet with parents and assist families to collaborate with school & other providersProvide services to include: a resource for parents & families, a mentor or coach for parents in need of support, an advocate for the needs of the parents and their families and a facilitator of parents groups to help educate and increase networking opportunities The above job functions are intended to describe the general nature of the position. QUALIFICATIONS REQUIRED:

High School Diploma Complete an internal training and pass a competency test on electronic clinical documentation software during the first eight weeks of employment.A valid driver's license and current auto insurance may be required.

QUALIFICATIONS PREFERRED:

Prefer minimum of 2 years navigating some level of system: Behavioral Health, DD Services, Education, and/or Juvenile Court

JOB SPECIFIC REQUIREMENTS:

Able to work in a team setting either at school or with their peers Consult with team members concerning situations and possible course of action Manages own schedule and works hours that follow program and Labor Board regulations
